GLP-1s and sleep: insomnia and fatigue on Ozempic, Wegovy, Mounjaro, Zepbound, and Foundayo

The short answer

Insomnia is not listed as a common side effect on the labels for Ozempic, Wegovy, Mounjaro, Zepbound, or Foundayo. Fatigue is. On Wegovy, 11% of people felt tired or weak, compared with 5% on placebo.1

When sleep gets worse on these medicines, the cause is often something around the drug: hunger or nausea at night, reflux, low blood sugar, or a routine that has changed. Look at each one, keep a steady bedtime, and talk to your doctor if it does not get better.

"Is it my GLP-1, or something else?"

I have a lot of friends on Ozempic, Mounjaro, and Zepbound. Most are thrilled with how they feel. But some have asked me the same thing: "I am exhausted all day, but I cannot sleep at night. Is it my medicine?"

My answer is always the same. It could be the GLP-1, but it could be other things too. You have to look at all of the things that affect your sleep, one by one.

What the labels say about sleep and fatigue

Ozempic and Wegovy are the same medicine (semaglutide). Mounjaro and Zepbound are the same medicine (tirzepatide). Foundayo (orforglipron) is a daily pill approved in April 2026.3 Here is what their official labels report.

Medicine Tired or weak Insomnia
Wegovy (same medicine as Ozempic) 11% vs 5% on placebo Not listed
Zepbound (same medicine as Mounjaro) 5% to 7% vs 3% on placebo Not listed
Foundayo (daily pill) Listed as common (5% or more) Not listed

These numbers come from weight loss trials. Ozempic and Mounjaro were studied in people with diabetes, at different doses, so their numbers can differ.1,2,3

So feeling tired is a real, known side effect. Not sleeping well is not on the list. That is why it pays to look at what else changed. For more on low energy, read why GLP-1s can make you tired.

Five things that can keep you up on a GLP-1

Cause 1

Hunger at night

Signs: you eat much less during the day, then wake up hungry or cannot settle down.

What to try: ask your care team about a small, protein rich snack in the evening, and keep regular meal times.

Cause 2

Nausea

Signs: you feel sick to your stomach in the evening, often after a dose increase.

What to try: eat smaller meals, stop eating a few hours before bed, and tell your prescriber if nausea keeps you up.

Cause 3

Reflux

Signs: burning in your chest or throat when you lie down. Reflux is listed as a side effect on the Zepbound and Foundayo labels.2,3

What to try: finish dinner earlier, raise the head of your bed, and ask your doctor or pharmacist about it.

Cause 4

Low blood sugar

Signs: you wake up sweaty, shaky, or with a racing heart. This is most likely if you also take insulin or a sulfonylurea for diabetes. On Zepbound, 10% of people who also took a sulfonylurea had low blood sugar, compared with 2% of people who did not.2

What to try: check your blood sugar if you can, and call your doctor. Your diabetes medicine may need to change.

Cause 5

A routine that has changed

Signs: new workouts, later meals, a moving bedtime, or more screen time at night.

What to try: keep the same bedtime and wake time, and do not push your workouts too hard while you adjust. See tired but cannot sleep.

What I tell friends to do

Stay as close to your routine as you can. If a supplement helps you relax and wind down, it can support that routine. Take it at the same time every night, before bed.

If you use melatonin, low dose is best. The last thing you want, when you already feel tired, is something that adds to your fatigue the next day.

There is research behind that. In adults over 50, a 3 mg dose of melatonin kept levels high into the next day, while 0.3 mg was enough to bring night levels back to normal.4 Read how much melatonin should I take to learn more.

  1. Keep a sleep diary for two weeks. Write down your dose day, meals, symptoms at night, and how you feel in the morning.
  2. Look for a pattern. Is it worse after a dose increase, a late meal, or a workout?
  3. Fix what you can. Earlier dinner, a steady bedtime, phone out of the bedroom.
  4. Ask your pharmacist before adding melatonin or any supplement.
  5. Talk to your prescriber if it lasts. Do not stop or change your medicine on your own.

Can I take melatonin with Ozempic, Zepbound, or Foundayo?

The labels for these medicines do not list melatonin as an interaction. But they do say these drugs can change how well other pills are absorbed, so care is needed with any medicine you take by mouth.1,2 That is why the best answer is to check with your pharmacist first.

Important for women on birth control pills

The Zepbound label advises women on birth control pills to switch to a method that is not a pill, or add a backup method, for 4 weeks after starting and for 4 weeks after each dose increase.2 Mounjaro is the same medicine. Ask your doctor what this means for you. In perimenopause or menopause? Read GLP-1s in menopause: night sweats, HRT, and sleep.

Are GLP-1s peptides?

Most of them are. A peptide is a short chain of amino acids, the building blocks of protein. Semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) are peptides made to act like natural gut hormones. That is one reason most of them are given as a shot.

Foundayo is different. It is a small molecule pill, not a peptide, and it can be taken with or without food or water.5

You may also see "research peptides" sold online, including products called retatrutide. These are not approved medicines, and no one checks what is in them. Read retatrutide and sleep to learn why that matters.

Talk to your doctor if

  • Your sleep has been poor for more than a few weeks.
  • You wake up sweaty, shaky, or with a racing heart.
  • Nausea, vomiting, or reflux keeps you up at night.
  • You snore loudly or stop breathing in your sleep.
  • You feel low, anxious, or not yourself.
